บทคัดย่อ:
เรื่อง แนวทางการพัฒนารูปแบบการจัดซื้อจัดจ้างการก่อสร้างอาคารโรงพยาบาล
ของคณะแพทยศาสตร์ในประเทศไทย

การวิจัยครั้งนี้มีวัตถุประสงค์เพื่อศึกษาสถานการณ์ปัญหาและอุปสรรคของการจัดซื้อจัด
จ้างการก่อสร้างอาคารโรงพยาบาลของคณะแพทยศาสตร์ในประเทศไทย ที่มีมูลค่า 100 ล้านบาทขึ้นไป
เพื่อวิเคราะห์แนวทางการแก้ไขปัญหาและอุปสรรคของการจัดซื้อจัดจ้างการก่อสร้างอาคาร
โรงพยาบาล และเพื่อเสนอแนวทางการพัฒนารูปแบบการจัดซื้อจัดจ้างการก่อสร้างอาคารโรงพยาบาล
โดยการสัมภาษณ์เชิงลึกผู้บริหารโรงพยาบาล บุคลากรทางการแพทย์ ผู้ปฏิบัติงานพัสดุ ของโรงพยาบาล
มหาวิทยาลัย ผู้ค้ากับภาครัฐ และผู้ที่มีส่วนเกี่ยวข้องกรมบัญชีกลาง ผลการวิจัยพบว่า ปัญหาและ
อุปสรรคที่เกี่ยวข้อง มีดังนี้1) ปัญหาเกี่ยวกับนโยบาย โครงสร้างและอ านาจหน้าที่ของคณะกรรมการ
ตามพระราชบัญญัติการจัดซื้อจัดจ้างและการบริหารพัสดุภาครัฐ พ.ศ. 2560 ที่มีการรวมอ านาจไว้ที่
กระทรวงการคลังเพียงองค์กรเดียว ควรมีการแก้ไขโครงสร้างและอ านาจหน้าที่มิให้มีการรวมศูนย์
อ านาจไว้ที่กระทรวงการคลังเพียงหน่วยงานเดียว 2) ปัญหาเกี่ยวกับความรู้ความเข้าใจในการ
ปฏิบัติงานของนักบัญชีภาครัฐ ซึ่งภาครัฐควรมีการประชาสัมพันธ์เชิญชวนผู้รับจ้างในบัญชีหรือผู้สนใจ
มาท าความเข้าใจในระบบการจัดซื้อจัดจ้าง 3) ปัญหาเกี่ยวกับการมีส่วนร่วมของประชาชนในการ
ตรวจสอบกระบวนการจัดซื้อจัดจ้างภาครัฐที่ขาดรูปแบบที่ชัดเจน ควรให้ประชาชนที่มีความรู้ได้เข้า
มามีส่วนในการตรวจสอบ 4) ปัญหาการบริหารงานที่มีสัญญาหลายสัญญาในเวลาเดียวกัน มีความ
ซับซ้อนและใช้ระยะเวลานาน ควรแบ่งงานออกเป็นหลายสัญญา เพื่อสะดวกในการบริหาร 5) ปัญหา
ของผู้ประกอบการที่ขาดประสิทธิภาพไม่สามารถที่จะท างานให้ส าเร็จลุล่วงไปได้ตามเวลาที่ก าหนด
ส่งผลให้ภาครัฐเกิดความเสียหาย ควรมีการตรวจสอบสถานที่ปฏิบัติงานจริงและความมีประสิทธิภาพ
6) ปัญหาการผูกขาดสินค้าและราคาในการจัดซื้อจัดจ้างแบบเจาะจงต้องระวังและค านึงถึงประโยชน์
สูงสุดของภาครัฐ โดยผลที่ได้จากการพัฒนารูปแบบการจัดซื้อจัดจ้างการก่อสร้างอาคารโรงพยาบาล
ของคณะแพทยศาสตร์ในประเทศไทยควรค านึงถึงความโปร่งใส คุณภาพ ประสิทธิภาพและด้านราคา
ที่เหมาะสม ข้อเสนอแนะในการวิจัยครั้งนี้ ควรมีการทบทวนการก าหนดคุณลัษณะ แบบรูป
รายละเอียด และขอบเขตงาน (TOR) เพื่อความเหมาะสมด้วยหลักความโปร่งใส วางนโยบาย
ให้ประชาชนได้มีส่วนร่วมในกระบวนการตรวจสอบ จัดท าคู่มือปฏิบัติงานให้กับผู้ที่เกี่ยวข้องในการ
ปฏิบัติงาน และควรจัดให้มีข้อตกลงคุณธรรมส าหรับการก่อสร้างที่มีวงเงินตั้งแต่ 100 ล้านบาทขึ้นไปข

Title Guidelines for the development of a procurement model for the
construction of a hospital building of the Faculty of Medicine in Thailand

This study aimed to study the problems and obstacles in procurement
and construction of hospital buildings of the Faculty of Medicine in Thailand worth
100 million baht or more, to analyze solutions to problems and obstacles in the
procurement and construction of hospital buildings, and to propose guidelines for
the development of the procurement model for the construction of hospital
buildings. The in-dept interview from hospital executive, medical personnel, parcel
operator of the university hospital, traders with the government, and people
involved in the Comptroller General's Department. The results revealed that the
Problems and obstacles are as follows: 1) Problems with the policy, structure, and
authority of the committee under the Public Procurement and Supplies Management
Act B.E. 2560, which has concentrated power in the Ministry of Finance as a single
organization, the structure and authority should be revised to prevent centralization
of power in the Ministry of Finance as a single agency 2) Problems with knowledge
and understanding of the work of government accountants which the government
should publicize to invite contractors in the account or those who are interested to
understand the procurement system 3) Problems with people's participation in
auditing the public procurement process that lacks a clear form, people with
knowledge should be involved in the audit 4) The problem of managing multiple
contracts at the same time is complicated and takes a long time should be divided
into several contracts for ease of administration 5) The problem of inefficient
entrepreneurs being unable to complete the work on time, resulting in damage to
the government sector, the actual work location and efficiency should be examined
6) The problem of monopoly on products and prices in specific procurement must
be careful and take into account the best interests of the government. The results of
the development of the procurement model for the construction of hospital
buildings of the Faculty of Medicine in Thailand should take into account
transparency, quality, efficiency and reasonable prices. Recommendations for this
research should include a review of the characteristics, profile, and scope of work.
(TOR) for suitability with the principle of transparency, laying policies for the public to
participate in the inspection process, preparing operating manuals for those involved
in the operation, and should provide morality agreements for construction with
a budget of 100 million baht or more.ค
