เรื่อง แนวทางการบริหารจัดการโรงพยาบาลในสถานการณ์โรคระบาดอุบัติใหม่
กรณีศึกษาโรงพยาบาลพระมงกุฎเกล้า

โรคระบาดอุบัติใหม่เป็นภัยอันร้ายแรงก่อให้เกิดผลเสียต่อชีวิตและเศรษฐกิจอย่าง
มหาศาล เมื่อเกิดการระบาดของโรคอุบัติใหม่ทุกครั้งการบริหารจัดการทางด้านสาธารณสุขทุกระดับมี
ความสำคัญอย่างมากเพื่อป้องกัน รักษาและควบคุมการแพร่ระบาดของโรคอย่างมีประสิทธิภาพ
การดำเนินการดังกล่าวต้องมีการปรับเปลี่ยนตามเทคโนโลยีและสภาพแวดล้อมในช่วงสถานการณ์
นั้นๆ งานวิจัยฉบับนี้จึงทำการศึกษาถอดบทเรียนการบริหารจัดการโรงพยาบาลพระมงกุฎเกล้าในช่วง
สถานการณ์การระบาดของโรคโควิด-๑๙ เพื่อวิเคราะห์การบริหารจัดการโรงพยาบาลที่สามารถ
นำไปใช้ในการบริหารจัดการสถานการณ์การเกิดโรคระบาดอุบัติใหม่ในอนาคตอย่างมีประสิทธิภาพ
และนำเสนอแนวทางการบริหารจัดการโรงพยาบาลรูปแบบใหม่ในสถานการณ์โรคระบาดอุบัติใหม่
กลุ่มเป้าหมายที่ดำเนินการศึกษา ประกอบด้วย ๓ กลุ่ม คือ ผู้ป่วย/ญาติผู้ป่วย ผู้ให้บริการทางการ
แพทย์และผู้บริหาร ทำการศึกษาตั้งแต่ ๑ ธันวาคม ๒๕๖๕ ถึง ๓๑ พฤษภาคม ๒๕๖๖ ดำเนินการ
วิจัยเชิงคุณภาพ (Qualitative Research) การวิเคราะห์เนื้อหา (Content Analysis) ร่วมกับการใช้
ข้อมูลเชิงพรรณนา (Descriptive Research)
ผลการวิจัยจากการถอดบทเรียนประเด็นตามหลักการบริหาร POSDCoRB ที่ใช้ได้ดีใน
การบริหารสถานการณ์ปกติมาดำเนินการวิเคราะห์ในประเด็นต่างๆ เริ่มต้นด้วย การวางแผน
(Planning) การจัดหน่วยงาน (Organizing) การจัดการด้านบุคคล (Staffing) การควบคุมสั่งการ
(Directing) การประสานงาน (Coordinating) การรายงาน (Report) และการจัดการด้านงบประมาณ
(Budgeting) แต่ทั้งนี้ในสถานการณ์การระบาดของโรคอุบัติใหม่ที่โรงพยาบาลต้องมีด้วยคือ
การบริหารอย่างต่อเนื่องโดยไม่สามารถหยุดการดำเนินงานเดิมทั้งหมด มารับมือเฉพาะสถานการณ์
และยังต้องกอบกู้สถานการณ์อย่างรวดเร็วอีกด้วย การบริหารในภาวะวิกฤติจึงมีข้อจำกัดเมื่อใช้
หลักการบริหาร POSDCoRB ผู้วิจัยจึงนำเสนอรูปแบบการบริหารในสถานการณ์วิกฤต ชื่อ PROMPT
model ใช้ในการบริหารโรงพยาบาลในสถานการณ์การเกิดโรคอุบัติใหม่ประกอบด้วย การเตรียมการ
(Prepare) การดำเนินการศึกษาทบทวน (Revision) การดำเนินการ (Operation) การวัดประเมินผล
(Measurement) การพักฟื้นหลังการปฎิบัติการ (Post Operation) ขั้นตอนสุดท้ายปิดจบสถานการณ์
(Termination) ซึ่งครอบคลุมตั้งแต่การเตรียมการ การรับมือสถานการณ์ การปรับเปลี่ยนตามปัญหา
ที่เปลี่ยนไปและเข้าสู่การปฏิบัติแบบใหม่ในที่สุดก

Title Guidelines for Hospital Management in Novel Pandemic Situations:
A Case Study of Phramongkutklao Hospital

The emergence of a novel pandemic disease poses a significant threat to
both human life and the economy. Thus, it is crucial to prioritise and implement
effective measures at all levels of public health management to prevent, treat, and
control the spread of such diseases whenever there is an outbreak of a new disease.
Management strategies must also adapt to advancements in technology and changes
in situations. This research aims to extract lessons learned from the management of
Phramongkutklao Hospital during the COVID-19 pandemic in order to analyse hospital
management practises that can be effectively applied in future outbreaks of new
pandemic diseases. Additionally, the study introduces an innovative model for hospital
management in the context of pandemic outbreaks. The target groups for this study
consist of three categories: patients/family members, medical staff, and administrators.
The research was conducted from December 1, 2022, to May 31, 2023, utilising
qualitative research methods, content analysis, and descriptive research
methodologies.
To guide the analysis in the context of the COVID-19 situation, the research
employs the "POSDCoRB model", which is typically utilised in managing normal
situations. This model encompasses various dimensions, namely Planning, Organising,
Staffing, Directing, Coordinating, Reporting, and Budgeting. However, it is essential to
recognise that during a pandemic outbreak, hospitals need to continuously manage
the situation without halting all existing operations and swiftly respond to and recover
from the crisis. Consequently, the limitations of the POSDCoRB model in crisis situations
become evident. In light of this, the research proposes an innovative management
model known as the "PROMPT model", specifically designed for effectively managing
hospitals in the face of new pandemic outbreaks. The PROMPT model consists of six
stages: Prepare, Revision, Operation, Measurement, Post-Operation, and Termination. ข
This comprehensive model covers all stages, ranging from preparedness to responsive
operations, in the face of changing circumstances during a novel pandemic outbreak.
Ultimately, the PROMPT model serves as a new paradigm for hospital management.ก
