บทคัดยอ
เรื่อง แนวทางการจัดระบบบริการสุขภาพเพื่อรองรับการเปนสังคมผูสูงอายุของประเทศไทย

ปจจุบันประชากรโลกไดเปลี่ยนผานสูประชากรสูงวัย ประเทศไทยมีผูสูงอายุสูงสุดเปน
อันดับ 2 รองจากประเทศสิงคโปรโดยในป2583 จะมีผูสูงอายุรอยละ 32.1 ของประชากรทั้งหมด
ประเทศไทยจําเปนตองเตรียมการเพื่อรองรับสถานการณที่คาดการณไวทั้งในดานการดูแลรักษา การ
บริบาลผูสูงอายุทั้งในดานสังคม สุขภาพ และสิ่งแวดลอมอยางเหมาะสมโดยมีเปาหมายใหผูสูงอายุที่มี
ภาวะ การวิจัยครั้งนี้มีวัตถุประสงคเพื่อวิเคราะหและประเมินสถานการณการใหบริการทางการแพทย
สําหรับผูสูงอายุตามหลักการบริหารจัดการระบบสุขภาพขององคการอนามัยโลกเพื่อระบุประเด็น
ปญหาในการใหบริการที่ควรไดรับการพัฒนา และเสนอแนวทางการปรับปรุงระบบการบริการทางดาน
สุขภาพใหสอดรับกับความตองการดานสุขภาพผูสูงอายุในประเทศไทย โดยการวิจัยครั้งนี้เปนการ
วิเคราะหขอมูลทรัพยากร และศักยภาพการจัดบริการดานสุขภาพแกผูสูงอายุในสถานบริการภาครัฐใน
พื้นที่นํารอง 5 จังหวัด ไดแก นครราชสีมา สุราษฎรธานี ขอนแกน เชียงรายและนนทบุรีภายใตระบบ
สารสนเทศทางสุขภาพที่มีในปจจุบันรวมกับการรวบรวมความคิดเห็น ขอเสนอแนะจากผูเชี่ยวชาญและ
ผูใหบริการดานสุขภาพแกผูสูงอายุจากหนวยงานตางๆ ทุกภาคสวนที่เกี่ยวของภายใตกรอบความคิด
ระบบบริการสาธารณสุขขององคการอนามัยโลกและวิเคราะหขอมูล ผลการวิจัยพบวา การจัดระบบ
บริการสาธารณสุขสําหรับผูสูงอายุที่มีภาวะพึ่งพิงยังไมครอบคลุมทุกพื้นที่ โดยพบการจัดบริการใน
ระดับโรงพยาบาลศูนย โรงพยาบาลทั่วไปและโรงพยาบาลชุมชน แตยังคงขาดการจัดบริการสาธารณสุข
ระยะกึ่งเฉียบพลัน ซึ่งสามารถกําหนดแนวทางการจัดระบบบริการสุขภาพสําหรับผูสูงอายุของประเทศ
ไทย ไดดังนี้1. การจัดระบบบริการสุขภาพผูสูงอายุควรเริ่มในทุกระดับตั้งแตครอบครัว ชุมชน จนถึง
สถานบริการสุขภาพและมีระบบสงตอและใหคําปรึกษาระหวางแพทยกับผูจัดการดูแล 2. การเตรียม
สถานบริการและพัฒนาผูดูแลผูสูงอายุโดยกําหนดคุณสมบัติและหลักสูตรสําหรับผูทําหนาที่ในระบบ
การดูแลผุสูงอายุ3. บูรณาการระบบขอมูลระหวางกระทรวง ไดแก ฐานขอมูลทะเบียนราษฎร
ฐานขอมูลสุขภาพ เปนตน พรอมทั้งกําหนดมาตรฐานความปลอดภัยของขอมูล 4. ควรสนับสนุนใหเกิด
การผลิต คิดคนนวัตกรรมใหมๆสําหรับดูแลผูสูงอายุ5. การบูรณาการงบประมาณเพื่อบริการดาน
สวัสดิการสังคมตลอดจนการออมเงินเขาสูกองทุนเพื่อชราภาพ 6. พัฒนารูปแบบและแนวทางการ
ปฏิบัติและการบริหารจัดการดานสุขภาพและสังคมของผูสูงอายุ โดยใหคณะกรรมการบริหารจัดการ
เครือขายสุขภาพอําเภอ บริหารจัดการ ติดตามและประเมินผลการดําเนินงาน อีกทั้งมีอํานาจในการ
บริหารจัดการกองทุนสําหรับผูสูงอายุใหเกิดประโยชนสูงสุด

The population of the world has changed to the ageing population. Thailand has
thehighest elderly of ASEAN, In2583the elderly people are32.1 percent of the total population.
To prepare health service system is necessary to accommodate the situation. To take care
elderly people, both in the social, health and environmental goals are appropriately. The
objective of research areto analyze and assess thehealth servicefor the elderlysituation based
on health systems management, the principles of the World Health Organization to identify
issues in service delivery should be developed. And to propose guidelines to improve the health
service system, provide a consistent to health needs of the elderly. In addition, the document
research, is the resources and potential to organize health services system for the elderly in
public sector services are analyze,Five areas of this study consist of Nakhon Ratchasima,
Suratthani, Khon-Kaen, Chiang Rai and Nonthaburi province. Under the health information
systems that are currently documents and shared with the gathering reviews. Suggestions from
the experts, and health care providers for the elderly from all agencies in related sectors are
reviews. By the public health service system, the conceptual framework of the World Health
Organization, and content analysis are used in this document.The result of this study are as
follow: the public health service system for dependent elderly people is not available in all
areas.Theservices found inthetertiary hospital, secondary hospital and primary hospital, lackof
subacute health care service.The guideline of health service system for elderly people is as
follows:1.The elderly health services system should start in all levels, the family. Community
health services, and there is a forwarding system and consultation between doctors to care
managers. 2.Preparation and development of care for ageing without qualifications and courses
for those acts in the administration system, consist of the elderly.3.For management information
system by integrated information system between Ministry of citizenship registration database
include database with health information and define information security standards. 4. Should
encourage the production of new innovation for elderly care.5. An integrated budget for welfare
of income and the Funding for elderly are sustainableto function. 6. Development model and
guidelines of health service system for elderly, District Health Board authorities are planning,
monitoring and evaluation ageing project encourages beneficialmanagement fundingfor elderly.
