เรื่อง แนวทางการสงเสริมบทบาทของนักจัดการสาธารณสุขชุมชนในการทำงานที่
เกี่ยวกับความมั่นคงดานสาธารณสุขของไทยอยางยั่งยืน

วัตถุประสงคการวิจัย 3 ประการ คือ 1) เพื่อศึกษาบทบาทของนักจัดการสาธารณสุขชุมชน
ในปจจุบันของไทย 2) เพื่อศึกษาวิเคราะหการเปนนักจัดการสาธารณสุขชุมชนอยางยั่งยืน และ 3) เพื่อ
ศึกษาแนวทางการสงเสริมบทบาทของนักจัดการสาธารณสุขชุมชนในการทำงานที่เกี่ยวกับความมั่นคง
ดานสาธารณสุขของไทยอยางยั่งยืน การวิจัยเปนการวิจัยเชิงคุณภาพ ดวยการรวบรวมขอมูลจากการ
การศึกษาเอกสารที่เกี่ยวของ และการสัมภาษณแบบเจาะลึก (In-depth Interview) กับผูใหขอมูลหลัก ซึ่งเปน
ตัวแทน อสม. โรงพยาบาลสงเสริมสุขภาพตำบล และชุมชน รวม 11 ราย เครื่องมือในการวิจัย คือ การ
สัมภาษณเชิงลึก การวิเคราะหขอมูลใชการตีความจากขอมูลที่เชิงคุณภาพ
ผลการวิจัย
ผลการศึกษบทบาทของนักจัดการสาธารณสุขชุมชนในปจจุบัน พบวา เกี่ยวของกับ
ประเด็นที่สำคัญ 3 ประการ คือ ปจจัยนำเขา กระบวนการ และ ผลลัพธในดานการสงเสริมสุขภาพ
และการปองกันโรค
ผลศึกษาวิเคราะหการเปนนักจัดการสาธารณสุขชุมชนอยางยั่งยืน บทบาทของนักจัดการ
สาธารณสุขชุมชน พบวา กระบวนการในการดำเนินงานดานการสงเสริมสุขภาพและการปองกันโรค
ใหกับประชาชนในพื้นที่มีความแตกตางกันไปในแตละโครงการและพื้นที่ที่ศึกษา ทั้งนี้ขึ้นอยูกับความ
ยากงายของสภาพปญหา ซึ่งกระบวนการในการดำเนินงานสามารถวิเคราะหออกเปน 2 กระบวนการ
คือ กระบวนการภายในโรงพยาบาลสงเสริมสุขภาพตำบล และกระบวนการประสานภาคี เครือขาย
และ อสม. ซึ่งสิ่งที่สงผลตอความสำเร็จและยั่งยืน คือ ระบบการทำงานในลักษณะของทีมเจาหนาที่
ของโรงพยาบาลสงเสริมสุขภาพตำบล และอาสาสมัครมีกระบวนการทำงานเชิงรุกรวมกัน ในลักษณะ
“ทีมสุขภาพในโรงพยาบาลสงเสริมสุขภาพตำบล” ในฐานะนักจัดการสาธารณสุขชุมชน
ผลการศึกษาแนวทางการสงเสริมบทบาทของนักจัดการสาธารณสุขชุมชนในการ
ทำงานที่เกี่ยวกับความมั่นคงดานสาธารณสุขของไทยอยางยั่งยืน พบวา ควรมีการสงเสริมบทบาท
3 ดาน คือ ดานบุคลากร ดานความเขมแข็งของชุมชน และดานภาคีเครือขายมีสวนรวม

Abstract
Title Guidelines for promoting the role of community health managers in working
on the sustainability of public health security in Thailand.

The three objectives of this research were: 1) to study the roles of
community health managers in Thailand; 2) to study and analyze sustainability of
community health managers; and 3) to study guidelines for promoting the roles of
community health managers in working on health security and sustainable public
health in Thailand. The research methodology used was a qualitative research by
collecting data from the study of relevant documents and in-depth interviews with
key informants who were representatives of the VHV, health promoting hospitals in
sub-districts and communities, totaling 11 cases. The research tool used was an indepth interview. The data analysis used was qualitative interpretations of the data.
Research Results
Result of the study of the current role of community health managers
was found to be related to three important issues: inputs, processes, and outcomes
in health promotion and disease prevention.
Result of the study and analysis of the role of being a sustainable
community health manager found that the process of health promotion operations
and disease prevention for people in the area varies in each project and study area
depending on the difficulty of the problem condition. The operation process can be
analyzed into 2 processes, which are the internal processes of the sub-district health
promoting hospitals, and the process of coordinating network partners and VHVs
(Village Health Volunteers). What affects the success and sustainability is the system
of working in the form of a team of staff of sub-district health promoting hospitals
and that the volunteers have a proactive work process together as a “health team in
sub-district health promoting hospitals” as community health managers.
Result of the study on the guidelines for promoting the role of
community health managers in working on the sustainability of public health security
in Thailand revealed that there are 3 roles that should be promoted, namely,
personnel, strength of the community, and the network partners involvement.
