เรื่อง การศึกษาโครงการจัดบริการคลินิกรูปแบบพิเศษในโรงพยาบาล

การจัดบริการคลินิกรูปแบบพิเศษในโรงพยาบาลภาครัฐจะเปนทางเลือกที่เหมาะสม
ในการแกปญหาความขาดแคลนทั้งดานบุคลากร งบประมาณ และทรัพยากรตางๆ สามารถเติมเต็ม
ความตองการของกลุมผูรับบริการที่ตองการบริการที่ดีเยี่ยม รวมทั้งยังสามารถลดความแออัดของการ
ใหบริการในระบบปกติสงผลใหความพึงพอใจตอการใหบริการสูงขึ้นดวย เพื่อสรางแนวทางตนแบบ
ในการจัดตั้งบริการรูปแบบพิเศษในโรงพยาบาลพระมงกุฎเกลา โดยเปนการวิจัยเชิงคุณภาพ โดยใช
วิธีการวิจัยเอกสาร (Documentary Review) ผลการวิจัย แผนดำเนินการในระยะแรก คือ การจัดตั้ง
หอผูปวยรูปแบบพิเศษตนแบบ จำนวน 2 หอผูปวย ในอาคารเฉลิมพระเกียรติสมเด็จพระนางเจา
สิริกิติ์พระบรมราชินีนาถ ชั้นที่ 15 และ 16 ในชื่อ “แผนกบริการพิเศษ” แตละหอผูปวยจะมีจำนวน
30 หอง รวมทั้งสิ้น 60 โดยใชแหลงเงินทุนจากเงินรายรับสถานพยาบาลโรงพยาบาลพระมงกุฎเกลา
และเงินสนับสนุนจากมูลนิธิโรงพยาบาลพระมงกุฎเกลา ติดตามผลการดาเนินงานโดยพิจารณาจาก
งบการเงินทุก 6 เดือน เพื่อปรับแผนการบริหารงานใหเหมาะสมอยูเสมอ และประเมินผลการ
ดำเนินงานเมื่อครบ 2 ปโดยหากมีรายไดสวนเกินจากตนทุนในแตละปเฉลี่ยตั้งแตรอยละ 10
ของเงินทุนตั้งตน พิจารณาดำเนินการตอไป และหากมีรายไดสวนเกินจากตนทุนในแตละปเฉลี่ยตั้งแต
รอยละ 30 ของเงินทุนตั้งตน พิจารณาขยายการใหบริการรูปแบบพิเศษในสวนอื่นๆ ตอไป
สรุปผลงานวิจัยและขอเสนอแนะ : โรงพยาบาลพระมงกุฎเกลามีศักยภาพในการ
จัดบริการรูปแบบพิเศษ เนื่องจากมีความพรอมดานตางๆ ซึ่งการจัดบริการรูปแบบพิเศษจะชวยเติม
เต็มความตองการของผูรับบริการ โดยเฉพาะอยางยิ่ง กลุมบุคคลสำคัญ กลุมผูมีประกันสุขภาพ
กลุมขาราชการและครอบครัวที่ตองการบริการที่สะดวกรวดเร็วกวาในระบบปกติ โดยยังสามารถ
ใชสิทธิเบิกคารักษาพยาบาลไดโดยควรจัดบริการรูปแบบพิเศษในลักษณะโครงการตนแบบ
เพื่อประเมินความสำเร็จ ปญหา และอุปสรรคตางๆ และนำขอมูลมาใชวางแผนสำหรับการดำเนินการ
เต็มรูปแบบในอนาคต

Abstract
Title : The Study to Find a Model of Special Clinic Service Arrangement in
Phramongkutklao Hospital
Field : Military

Providing specialized clinic services in the government hospitals would
be the precise choice to solve the shortage of personnel, budget and resources. The
specialized clinic able to fulfill the needs of service recipients who want excellent
service. It can also reduce the congestion of services in the normal system.
Accordingly, the satisfaction of the service is also higher. and To create a model
guideline for the establishment of special clinic services in Phramongkutklao Hospital
Methods: This is a qualitative research by using a documentary review method.
Results: The action plan in the first phase is to establish two special-model wards in
the building in honor of Her Majesty Queen Sirikit, the 15th and 16th floors, under
the name "Special Service Department". Each ward will have 30 rooms with 60 rooms
in total. The project funding sources will be from the contribution of
Phramongkutklao Hospital income budget and financial support from
Phramongkutklao Hospital Foundation. The six-month financial statement will be
considered in order to consistently establish the appropriate management plan. The
two years of project evaluation will be performed and cogitated. If there is a surplus
from costs each year, on average from 10% of the initial capital, consider proceeding.
Moreover, if there is an average annual cost surplus of more than 30% of the initial
capital, consider expanding the special service in other areas in the future.
Conclusion & Suggestion: Phramongkutklao Hospital has the potential to
provide the special clinic services due to the willingness in various aspects. The
special clinic services will help fulfill the needs of service recipients, especially group
of important people, health insurance group, civil servants and families who want
services that are more convenient and faster than in the normal system.
