การแพทย์ทหาร ได้รับการกำหนดให้เป็นประเด็นยุทธศาสตร์หลัก ๑ ใน ๖ ของประชาคม
การเมืองและความมั่นคงอาเซียน (ASEAN Political-Security Community ; APSC) โดยมติของการ
ประชุมรัฐมนตรีกลาโหมอาเซียน ครั้งที่ ๔ (The 4th ASEAN Defense Ministers’ Meeting : ADMM)
ณ กรุงฮานอย ประเทศเวียดนาม เมื่อวันที่ ๑๑ พฤษภาคม พ.ศ. ๒๕๕๓ อันประกอบด้วย การปฏิบัติการ
รักษาสันติภาพ, การช่วยเหลือด้านมนุษยธรรมและการบรรเทาภัยพิบัติ, ความมั่นคงทางทะเล, การ
ต่อต้านการก่อการร้าย, การแพทย์ทหาร และการปฏิบัติการทุ่นระเบิดเพื่อมนุษยธรรม (เพิ่มเติมในการ
ประชุมรัฐมนตรีกลาโหมอาเซียน ครั้งที่ ๗ (The 7thADMM) ณ กรุงบันดาร์เสรีเบกาวัน ประเทศบรูไน
เมื่อวันที่ ๖ พฤษภาคม พ.ศ. ๒๕๕๖) จึงได้มีการจัดตั้งคณ ะทำงานผู้เชี่ยวชาญเฉพาะด้าน
(Experts’WorkingGroups:EWGs) ขึ้นมาเพื่อดำเนินกิจกรรมความร่วมมือในแต่ละด้าน ทั้งนี้ ประเทศ
ไทยได้รับเกียรติให้เป็นประธานร่วมกับประเทศรัสเซียในคณะทำงานผู้เชี่ยวชาญเฉพาะด้านการแพทย์
ทหาร (Experts’WorkingGroupon Military Medicine:EWG-MM)ตั้งแต่ปี พ.ศ. ๒๕๕๗ – ๒๕๕๙ เป็น
เวลา ๓ ปี ซึ่งไทยจะต้องเป็นเจ้าภาพในการจัดกิจกรรมการประชุมเชิงปฏิบัติการ การสัมมนาแลกเปลี่ยนความรู้และเทคโนโลยีด้านการแพทย์ทหาร รวมทั้งการฝึกร่วมผสมของหน่วยแพทย์ทหารเฉพาะกิจ
นานาชาติ ในการให้การช่วยเหลือด้านมนุษยธรรมและบรรเทาภัยพิบัติ อีกหลายครั้งนับจากนี้ไป
ผู้วิจัยจึงมีความสนใจที่จะทำการศึกษาวิจัยในเรื่อง “แนวทางพัฒนากิจการด้านการแพทย์
ทหาร เพื่อเตรียมความพร้อมในการเข้าสู่ประชาคมการเมืองและความมั่นคงอาเซียนของกองทัพไทย”
เพื่อรวบรวมองค์ความรู้ แนวคิดของประชาคมการเมืองและความมั่นคงอาเซียน ภารกิจ การจัด หลักนิยม
และการดำเนินงานด้านการแพทย์ทหาร ในกองทัพไทย ตลอดจนข้อคิดเห็นจากผู้เชี่ยวชาญในประเด็น
ดังกล่าว เพื่อเป็นแนวทางในการปรับบทบาท พัฒนาองค์กร และเตรียมรับการเปลี่ยนแปลงด้าน
การแพทย์ทหาร อันจะยังผลให้เกิดความพร้อมที่จะปฏิบัติภารกิจของตนเอง หน่วยงาน และนโยบายของ
รัฐที่จะก้าวเข้าสู่การเป็นประชาคมอาเซียนโดยสมบูรณ์ในปี พ.ศ. ๒๕๕๘ ได้อย่างมีประสิทธิภาพ
ผู้วิจัยได้ดำเนินการวิเคราะห์สภาพแวดล้อม ด้วยการ SWOT Analysis และสัมภาษณ์เชิง
ลึก ผู้บังคับบัญชาในสายการแพทย์ทหาร จากสำนักงานแพทย์ทหาร กรมยุทธบริการทหาร (สนพ.ยบ.
ทหาร), กรมแพทย์ทหารบก (พบ.), กรมแพทย์ทหารเรือ (พร.) และกรมแพทย์ทหารอากาศ (พอ.) จน
ทราบแนวความคิดในการดำเนินงานด้านนโยบาย ในการพัฒนาปรับปรุงหน่วยและกำลังพลสายแพทย์
ทหาร ตลอดจนปัญหาอุปสรรค ข้อขัดข้องในด้านต่างๆ พบว่า หน่วยงานสายแพทย์ทหารในกองทัพไทย
แต่ละหน่วยต่างก็มีศักยภาพด้านเทคโนโลยีทางการแพทย์ในระดับสูง และมีบุคลากรด้านการแพทย์ทหาร
ที่มีขีดความสามารถสูงเป็นจำนวนมาก สามารถที่จะเป็นผู้นำด้านการแพทย์ทหารในภูมิภาคอาเซียนได้
(เว้นแต่สนพ.ยบ.ทหาร ที่มีโครงสร้างอัตราการจัดค่อนข้างจำกัด) แต่ยังขาดจุดเชื่อมต่อประสานงานที่
เหมาะสมและมีประสิทธิภาพ ที่จะรวมขีดความสามารถของหน่วยต่างๆ เข้าด้วยกันอย่างมีเอกภาพ และมี
ทิศทางเดียวกัน จึงเห็นควรที่จะมีการจัดตั้งหน่วยงานเฉพาะกิจ เพื่อประสานงานด้านการแพทย์ทหาร
ระหว่าง ๓ เหล่าทัพ และรองรับการทำงานในวาระที่ไทยเป็นประธานร่วมกับรัสเซีย ใน คณะทำงาน
ผู้เชี่ยวชาญเฉพาะด้านการแพทย์ทหาร (Experts’WorkingGroupon Military Medicine:EWG-MM)
โดยมีเป้าหมายที่จะพัฒนาเป็นศูนย์ประสานงานการแพทย์ทหาร และการบรรเทาภัยพิบัติของอาเซียน
อย่างถาวร ในระยะต่อไป ซึ่งจะต้องมีกิจกรรมการจัดประชุมทางไกล (Teleconference), การประชุมเชิง
ปฏิบัติการ และการสัมมนาแลกเปลี่ยนความคิดเห็นกับผู้แทนแพทย์ทหารของชาติสมาชิกในอาเซียนเป็น
ประจำ ในการหาแนวทางพัฒนาบทบาท ภารกิจ หลักนิยมด้านการแพทย์ทหารในอาเซียนร่วมกัน ยกระดับมาตรฐานของแต่ละชาติสมาชิกให้อยู่ในระดับเดียวกัน หรือใกล้เคียงกัน และสามารถระดมความ
ช่วยเหลือในการแก้ไขสถานการณ์วิกฤต ภัยพิบัติ โรคระบาดต่างๆ ก่อให้เกิดความร่วมมือกันอย่างแน่น
แฟ้น เป็นการนำพลังอำนาจด้านการทหาร และความมั่นคงมาใช้ในเชิงสร้างสรรค์ เพื่อให้เกิดสันติภาพ
ความสงบสุขอย่างแท้จริง อันเป็นการบรรลุวัตถุประสงค์ของการเป็นประชาคมการเมืองและความมั่นคง
อาเซียนนั่นเอง
ABSTRACT
Title : Preparedness for the ASEAN Political-Security Community (APSC)
on the Military Medical aspect of the Royal Thai Armed Forces

July2014Military medicine has been designated as one of the six main
strategic issues of the ASEAN Political-Security Community (APSC) by the
conclusion of The 4th ASEAN Defense Ministers’ Meeting (ADMM) in Hanoi,
Vietnam on May 11th , 2010 which comprised of Peacekeeping Operation (PO),
Humanitarian Assistance and Disaster Relief (HA-DR), Maritime Security (MS),
Anti-Terrorism (AT), Military Medicine (MM) and Humanitarian Mine Operation
(HMO). (The last has been added by The 7th ADMM in Bandarseri Begawan,
Kingdom of Brunei on May 6th , 2013.) Therefore, Experts’s Working Groups
(EWGs) were set up to undertake each specific strategy by seeking cooperations among nation members. Thailand has been honored together with
Russia to co-chair the Experts’ Working Group on Military Medicine (EWG-MM)
from 2014-2017 (3 years term). Along this period, we shall host a number of
conferences and seminars for exchange of knowledge and military medical
technologies, and by the end of the term, organize a multinational task force
military medical exercise focusing on Humanitarian Assistance and Disaster
Relief (HA-DR) in Thailand.
Therefore, I am interested in launching a research study on the topic
; “Preparedness for the ASEAN Political-Security Community (APSC) on the
Military Medical aspect of the Royal Thai Armed Forces” for the purposes of
gathering information and ideas of the APSC along with the backgrounds,
organizing and missions of the military medicine directorates in the Royal Thai
Armed Forces, topping up with comments from the experts in this field in order to analyze and present a reasonable proposal for the adjustments and
reorganizing processes of the Thai military medical units for the preparedness
to become an effective partner of the APSC by the year 2015.
In this studies, the environmental status had been concluded with
SWOT analysis method and together with information from in-depth interviews
with commanding authorities from The Medical Office of The Royal Thai
Armed Forces Headquarter (RTARF HQ), Directorates of Military Medicine of the
Royal Thai Army (RTA), Royal Thai Navy (RTN) and The Royal Thai Air-force
(RTAF) concerning policies, ideas for developing the unit and human resources
including problems or obstacles. The facts reveal that all of the military
medical organizations of the three Royal Thai Armed Forces (except for The
Medical Office of The RTARF HQ) have high technological potentials with
sufficient number of high standard medical personnel toback up Thailand’s
leading role in ASEAN region, but in contrary, we’re lacking of suitable and
effective joint connecting point that will unify and concentrate all of the units’
strength together on the same direction. For this reason we shall establish a
task force office or committee to connect between the three medical
organizations and support Thailand’s co-chair role with Russia in the
Experts’WorkingGroupon Military Medicine(EWG-MM). In the near future, this
unit will be transformed into a permanent ASEAN Military Medicine
Coordination Center (AMMCC). The regular activities in this center will be
teleconferences, operational meetings and seminars among the military medical fellows & representatives of the member nations in order to develop
and refine the ASEAN military medical roles, missions and doctrines to set the
new high standard of practice. In case of crisis from natural disasters or
epidemic diseases, this center will act promptly to recruit help and medical
support from neighboring and oversea countries to mitigate and relieve the
situation. These close coordination will surely bring peace and harmony
among the ASIAN region. Using the military power in such a creative way,
we’re truly achieved the goal of the ASEAN Political-Security Community!
