เรื่อง แนวทางในการพัฒนารูปแบบการบริหารจัดการระบบสุขภาพ โดย
คณะกรรมการพัฒนาคุณภาพชีวิต และระบบสุขภาพอ าเภอ ศึกษาเฉพาะ
กรณีอ าเภอเมือง จังหวัดสมุทรปราการ.

จากสภาพปัญหาของประชาชนคนไทย รัฐบาลได้ออกระเบียบส านักนายกรัฐมนตรี
ว่าด้วยการพัฒนาคุณภาพชีวิตระดับพื้นที่ พ.ศ.2561 ซึ่งก าหนดให้มีคณะกรรมการนโยบายพัฒนา
คุณภาพชีวิต ระดับพื้นที่ และคณะกรรมการพัฒนาคุณภาพชีวิตระดับอ าเภอ (พชอ.) เพื่อให้เกิดการ
ขับเคลื่อนการพัฒนาคุณภาพชีวิตของประชาชนในระดับพื้นที่อย่างเป็นระบบ และมีประสิทธิภาพ
โดยมีการบูรณาการร่วมกันระหว่างหน่วยงานภาครัฐ ภาคเอกชนและภาคประชาชน เพื่อตอบสนอง
ประเด็นยุทธศาสตร์ชาติข้อที่4 ด้านการสร้างโอกาส ความเสมอภาคทางสังคม และยุทธศาสตร์ชาติ
ข้อที่5 ด้านการสร้างการเติบโตบนคุณภาพชีวิตที่เป็นมิตรกับสิ่งแวดล้อม ซึงผู้วิจัยมีความเห็นว่า
พชอ. ที่รัฐก าหนดไม่น่าจะเป็นแนวทางในการบริหารจัดการที่เหมาะสม ส าหรับพื้นที่อ าเภอเมือง
จังหวัดสมุทรปราการ จึงก าหนดวัตถุประสงค์ในการวิจัย เพื่อศึกษาวิเคราะห์ถึงความเหมาะสม
ในการจัดตั้งคณะกรรมการการพัฒนาคุณภาพชีวิต และเสนอแนวทาง เพื่อการพัฒนารูปแบบการ
บริหารจัดการระบบสุขภาพ ศึกษากรณี อ าเภอเมือง จังหวัดสมุทรปราการ รูปแบบการวิจัยเป็น
เชิงคุณภาพ อิงกรอบการบริหารจัดการระบบสุขภาพ 6 Building Blocks ของ WHO เน้นการ
สัมภาษณ์กลุ่มเป้าหมายที่เกี่ยวข้อง จากการศึกษาวิจัยพบว่าการพัฒนารูปแบบการบริหารจัดการ
ของ พชอ.กรณีอ าเภอเมืองฯ จังหวัดสมุทรปราการ ให้ความส าคัญกับโครงสร้างการบริหารจัดการ
ที่มีนายอ าเภอ เป็นประธานกรรมการ และสาธารณสุขอ าเภอ หรือองค์กรปกครองส่วนท้องถิ่นเป็น
เลขานุการ และควรมีโครงสร้างสารสนเทศด้านสุขภาพที่มีองค์กรปกครองส่วนท้องถิ่นเป็น
เลขานุการ และสาธารณสุขอ าเภอเป็นผู้ช่วยเลขานุการ เพื่อการบริหารจัดการทั้งในด้านทรัพยากร
บุคคล ระบบงานบริการสุขภาพ ระบบยา และเวชภัณฑ์ให้ตรงตามประเด็นปัญหาของบริบทของ
หน่วยบริการปฐมภูมินั้นๆ และยังเน้นถึงระบบการคืนข้อมูลกลับสู่ประชาชน เพื่อการพัฒนา
ที่ยั่งยืน ส่วนในเรื่องการคลังด้านสุขภาพเสนอให้ พชอ. ต้องมีงบประมาณเป็นของตนเอง เพื่อใช้ใน
การขับเคลื่อนงานในทุกมิติของการพัฒนาคุณภาพชีวิตประชาชน

Title : Guidelines for the development of the Health Systems Management Model
by the committee on the Improvement of the Quality of Life at the Area in
2018. Case Study in Muang District, Samutprakarn Province

To respond to national strategic issues no.4. Opportunities and Social Equality and
National Strategy issues no. 5. Growth and Quality of Life for solution the problem of Thai
people. For the promotion and development of the quality of life of Thai people in the area
systematically and efficiently, with the integration of public, private and public sectors of the
district, the Government has issued the Prime Minister's Regulation on the Improvement of the
Quality of Life at the Area in 2018. The researcher, the director of Smut Prakarn center hospital
where are health services, is of the opinion that the committee in issued the Prime Minister's
Regulation on the Improvement of the Quality of Life at the Area in 2018. Government will not
be the appropriate committee and management for the Samut Prakarn district. So the purpose of
this research is to analyze the appropriateness of setting up a committee for quality of life
development and proposing a guideline for the development of the health system management
model for Muang district, Samut Prakarn province. The research model is qualitative that the
concept based on WHO's 6 Building Blocks. Focus on targeted interviews. From the study found
that the most of interviews have agree with committee. They are offer to the management
structure that the district commissioner as Chairman and the district public health department or
local government organization as a secretary. About informatics Health System, there should be a
health information structure with local administration as a secretary, the district public health
department is an assistant secretary for human resource management, health services medication
and medical supplies meet the issues of the context of the primary care unit are the offer of
development of the management model. It also emphasizes the return of information to the people
for sustainable development. In Health Finance, Offers that the committee must have his own
budget to use in driving all aspects of the development of people's quality of life.
